Forms

In September 1998, MSO released a CD-ROM containing our manuals, forms and endorsements. Each member company received a customized version of the CD-ROM, which includes forms that are specific to that company, as well as the Adobe Reader program. The forms and manual pages may be viewed or printed by using this program.

Company specific forms, manual pages, etc., that were not developed by MSO are not included on the CD-ROM. These can be included for any company who wishes to have them put on the CD. A sample needs to be sent on disk to our office, and we will make sure to include it on the next updated version. Those companies who have their own forms on file at MSO, will notice an extra folder on the CD-ROM named after that company. This folder contains those company-specific forms. It is important to remember that if you have given us your forms for the CD-ROM, you must make sure that you send us any changes that occur, so the forms will be up-to-date.

Changes cannot be made to the CD-ROM forms. Any forms requiring "typing entries" must be printed and then typed.
